THE derailment of a GWR two-car DMU at Paddington on June 16 caused travel chaos for thousands of passengers over several days.

Unit No. 166124 was working the 16.25 Reading depot-Paddington e.c.s. and approaching the station towards platform 1 and 2 when a category A SPAD (signal passed at danger) occurred near Westbourne Road bridge.
